NEW INFORMED CONSENT AND HEALTH FORM TO BE SIGNED ⛑️
I know this part is very boring but the health and waiver form has been updated - a few less boxes to fill out, the addition of an emergency contact (I suddenly realised who would I call if something happened to you in class?!) and updated wording in the terms and conditions. It really is very important for everyone to fill this out, and if you refuse to sign the form sadly you cannot participate in class. If you have any issues with the form please contact me to discuss.
This form is now updated from 01.08.25, so if anyone has signed a form before this date, you will have to complete this new one.
